I agree that my decision to participate in A Thousand Leaves Beginners Herbal Program (hereafter referred to as “Program”) with LeAnn Averell & Susan Marynowski (henceforth referred to as the "Facilitators") is a personal decision. No one has made any promises or warranties as to the results or benefits I will receive or as to any specific results I will obtain from my participation in this Program. I understand that this Program is not therapy nor medical counseling or treatment, and is not a substitute for the treatments or services ordinarily provided by health care professionals for physiological or psychological complaints. If I desire medical attention, I will seek it from a licensed provider. I accept full responsibility for any preparations made as part of the participation in this Program. 

I understand that the Program is for my personal education and benefit only and may not be appropriate for others. I understand that information shared in the Program is strictly confidential and can not be given out unless I sign a release form.

I understand that all content materials used in this Program are confidential and proprietary in nature and other than for my own personal use, I will have no future right to these materials or processes in any way without express training, written permission, and licensing by the Facilitators.

I affirm that I have answered all questions on this form honestly and fully disclosed all the information requested. I assume all risk associated with participating in this Program and release the Facilitators, A Thousand Leaves, LLC, and any other person associated with the aforementioned persons and entities (collectively, the “Released Parties”) from, and shall defend, and indemnify, and hold them harmless from and against, all liabilities, claims, actions, losses, causes of action, and costs in any way arising out of my participation in the Program. I release the Released Parties from any liability for injuries due to their negligence occurring now or in the future, during or after my participation in this Program.

Refund Policy:

If payment has been made and you decide that you can’t take the course after all, we will provide you with a full refund minus a $75 processing fee as long as you notify us within one week of purchase, as long as none of the course has been completed. No refunds will be issued after one week from start date. All requests must be sent via email in writing.

I CONFIRM THAT I HAVE READ AND UNDERSTAND THIS AGREEMENT FULLY AND INTEND TO BE LEGALLY BOUND BY IT.
